What is MAT 1 Form for?
This form is created by the Social Security System to notify the employer that a female employee is expecting a baby.
When is MAT 1 Form Due?
The female employee must file this form at least two months before the period of conception. The employer in their turn must file the notification within 15 days from the date when the employee has submitted MAT 1 Form.
Is MAT 1 Form Accompanied by Other Documents?
Yes, an applicant must attach Ultrasound Report or Pregnancy Test to the Maternity Notification. One of these documents must be provided to the employer. Moreover, you may be required to attach MAT 2 Form that is known as the Maternity Reimbursement.
What Information do I Include in MAT 1 Form?
The female applicant must provide the following information in the form: name (surname, given name, middle name), social security number, type of membership (self-employed, voluntary, employed or separated and the date of separation), home address, the date when the baby is expected, signature and date of filing. There is the part that must be completed by employer. In this part such information must be indicated: employer ID number, name, address, name of employer’s authorized representative, date and signature. The next part must be filled in by SSS only: signature and the date when the form was received and processed.
